Establishment of LC-MS/MS Analysis Approach of Folic Acid and Its Metabolites in Clinical Samples of Autistic Children

Folate and its metabolic derivatives serve as crucial cofactors in human metabolomics, imbalances with these metabolites marks various disease conditions such as kidney/liver damages and malfunctions. Therefore, quantification of folate and folate metabolite panels is crucial for clinical diagnosis and treatment. In this study, we developed an LC-MS method based on multiple reaction monitoring (MRM) for sensitive quantification of folate metabolite panel. In brief, metabolite is extract from 50 µL of plasma/serum via protein precipitation and analyzed by using an LTQ XL™ mass spectrometer coupled to an Vanquish HPLC. We demonstrated accurate quantification of multiple folate metabolites in the folate metabolite panel, including Folic acid (FA), Folinic acid (5-formyl tetrahydrofolic acid), DL-Homocysteine, L-Methionine, S-adenosylmethionine (SAM), S-(5'-Adenosyl)-L-homocysteine (SAH), and 5-methyltetrahydrofolate (5mTHF). Stable-isotope internal standards of the above-mentioned compounds are utilized in the LC-MRM method in compliance with existing clinical mass spectrometry regulations, CLSI-62A. Additionally, via rigorous optimization in sample preparation, MS ionization, and ion optics, the LC-MRM methods showed a significantly improved limit of quantification compared to reported LC-MS folate analysis methods. This method will be employed in an ongoing autism phase-2 clinical trial.
