Steroid profiling of biological samples by LC-MS/MS

Steroid hormones are endogenous compounds that are synthesized from cholesterol by a series of enzymatic reactions in the adrenal cortex, gonads, and placenta. They are involved in many biological processes and play important roles in many diseases. Thus, the comprehensive analysis of those steroids in various biological samples has a significant impact on disease diagnosis or health monitoring.
The aim of this study is to develop an efficient, sensitive and prompt analytical method for detecting steroid hormones in serum and urine. However, detection of steroids in serum and urine has a number of technical difficulties such as high lipid contents, huge amounts of protein and high contents of conjugated steroids. To circumvent these difficulties, I optimized the sample pretreatments for serum and urine, which involve the enzymatic hydrolysis, removal of proteins, and purification by solid-phase extraction. Quantitative analysis of steroids was performed with multiple reaction monitoring in positive-ion mode LC-MS/MS. The present method allowed 26 steroids from serum, and 27 steroids (after enzymatic hydrolysis) from urine to be qualified or quantified at a time. In addition, thought these analyses, we succeeded, for the first time, in identifying six novel steroids in serum.
