原発性アルドステロン症診療における質量分析への期待

Hypertension has a high morbidity that approximately one third of individuals is affected.  Hypertension causes various complications such as stroke, myocardial infarction, arrhythmia, renal dysfunction, etc. Primary aldosteronism, caused by aldosterone hypersecretion from adrenal gland, is a leading cause of secondary hypertension, occupying supposedly 5 to 20 % of all hypertension cases. Selective adrenal venous sampling is a criterion standard to diagnose the localization of aldosterone excess because morphological diagnosis by MDCT and MRI is not reliable. Precise detection of the hypersecretion enables curative, local treatment, eliminating the need for anti-hypertensive medication. Although selective and super-selective adrenal venous sampling to measure aldosterone and cortisol content in the adrenal gland is the only way to detect the site of aldosterone hypersecretion, arrangement of many blood samples are time consuming. Mass spectrometry analysis combined with angiographic display system is ongoing in industry-academia collaboration between Shimadzu and Tohoku University. Development of the system will enable low invasive interventional radiological treatment utilizing rapid aldosterone analysis.
