The screening of vitamin D deficiency in neonatal infants, which is based on the blood 25-hydroxyvitamin D3 [25(OH)D3] quantification, is important for the early detection, diagnosis and health risk assessment of several diseases. In the LC/ESI-MS/MS assay of 25(OH)D3 in the limited volume of samples, the insufficient sensitivity often becomes a major problem. Another problem in the 25(OH)D3 assay is the potential interference from its inactive epimer, 3-epi-25(OH)D3, leading to overestimation of the true 25(OH)D3 concentrations. By derivatization with DAPTAD, which is a new Cookson-type reagent developed by the authors, the detectability of 25(OH)D3 was increased 30 times. The derivatization was also useful for the LC separation of 25(OH)D3 from its 3-epier (Rs 1.5). In this presentation, the author demonstrates the applicability of DAPTAD and its isotopomers (2H3- and 2H6-DAPTAD) for the analyses of 25(OH)D3 and related metabolites in neonatal blood samples, and also for the diagnosis of neonatal vitamin D deficiency.
